In the event that we do not receive the required notice for adjustments and cancellations the following fees will be billed out to you I further understand that the procedure works as follows: • A telephone call must be made at least 24 hours prior to scheduled appointment to avoid a missed appointment fee. • Notification given less than 24 hour prior to appointment time will result in 50% of your service charge. • Notification less than 24 hours of ANY and all symptoms of COVID-19 or other communicable conditions will not be charged fee. • Failure to show up for your appointment will result in a full service amount.
